Members 13 years of age & under will require parental consent. A COPPA request form must be filled out to retrieve a one-time activation code for the initial log-in.

Where do I get a Parental Consent Form?

Please complete the COPPA Request Form and send us a copy to receive an activation code for the account. The form can be wet signed or electronically signed.

Please email the signed form to support@visionmedia.com.

Subject: COPPA Request Form

1. Please provide your name, the COPPA member's name, and a valid email address on the COPPA Request Form. A valid email address must be provided in order to receive the activation code. We collect this information in order to verify parental consent and communicate with you regarding your child’s use of the service.
2. Once the COPPA Request form is received, we will send you a one-time activation code by email.

How do I activate a COPPA account?

1. Log in to Screener Passport Movies from a browser. 
2. Enter the "Activation Code" field under Parental consent required.
3. You should now be logged in to Screener Passport. The activation code is only required once for the account, future logins will not need the activation code.
